OBJECTIVES

The objective was to estimate the budget impact of olaparib as the first oral available alternative as maintenance treatment of adult patients with platinum-sensitive relapsed BRCA-mutated (germline and/or somatic) high grade serous epithelial ovarian cancer who are in response (complete or partial) to platinum-based chemotherapy in the public healthcare system in Mexico, in order to achieve treatment objectives aiming to delay new symptoms, diminish toxicity of subsequent chemotherapy and increase quality of life

METHODS

A budget impact analysis was conducted to evaluate olaparib from the Mexican public payer perspective. Currently, these patients are treated with platinum-based chemotherapy followed by “watch and wait” approach for maintenance, therefore the comparator in this study was placebo. A time horizon of 5 years was considered. Only direct medical costs were included, considering drug acquisition and adverse events management. The analysis was developed following the local guidelines. A sensitivity analysis was also conducted to validate the robustness of the results. All costs are expressed in 2017 US dollars.

RESULTS

A small number of patients who are candidate to receive olaparib was estimated. Considering an initial market share of 5% and subsequent annual uptakes of 5% the introduction of olaparib to the Mexican public healthcare system would translate into an estimated incremental budget impact of 0.3% in the first 5 years, equivalent to USD$2.3 million. The sensitivity analysis confirmed the base case results, showing that the introduction of olaparib to the public health institutions won’t suppose a significant increase in their budget.

CONCLUSIONS

This model showed that the introduction of olaparib is likely to have minimal budget impact in the public healthcare system in Mexico, representing an affordable, innovative therapeutic option for patients with platinum-sensitive relapsed BRCA-mutated (germline and/or somatic) high grade serous epithelial cancer who are in response (complete or partial) to platinum-based chemotherapy.
