USE OF ANTIHYPERTENSIVE MEDICATIONS IN DIABETIC AND NON-DIABETIC CHRONIC KIDNEY DISEASE AND HEMODIALYSIS PATIENTS- A DUE STUDY

OBJECTIVES:   To study the prescribing pattern of antihypertensive medications in diabetic and non-diabetic patients of chronic kidney disease and hemodialysis patients. METHODS:   A retrospective observational study was conducted in patients diagnosed with Chronic Kidney Disease (CKD) in a tertiary care hospital over a period of one year. The patients diagnosed under the ICD-10 with diagnostic codes 18.3 to 18.6 were included in the study. The information regarding demographic details and antihypertensive drugs prescribed of Anatomic Therapeutic Chemical (ATC) classification in both diabetic and non-diabetic CKD patients were collected from the patient’s case records. The data was compiled and subjected to statistical analysis.  RESULTS:   Total of 562 case records were analyzed. Of the 69% of end stage renal disease patients, it was found that 37% of the patients were on dialysis. CKD was more common in males, belonged to the age group of 40-60 years followed by 60 – 69 years. The overall mean age was 54.40±13.64 years, 93% had a history of hypertension, and 45% had diabetes. The overall mean BMI was 23.12±4.57kg/m2 and HbA1c was 6.86±2.66%. The major anti-hypertensive drugs prescribed in CKD patients with diabetes were calcium channel blockers (CCBs) (79%) with both amlodipine and Cilnidipine accounting for 41% followed by diuretics (59%) with furosemide (87%) and centrally acting alpha agonists (43%) with clonidine (89%) and moxonidine (11%). In non-diabetic CKD patients, CCBs (75%) with amlodipine (48%), diuretics (46%) with furosemide (91%) and centrally acting alpha agonists (45%) with clonidine (93%) were the majorly prescribed drugs to treat hypertension in different stages of CKD and hemodialysis patients. CONCLUSIONS: CCBs, Diuretics and Centrally acting alpha agonists were the major class of antihypertensive drugs used to treat hypertension in both diabetic and non-diabetic CKD patients across all the stages and were followed by Beta blocker and Alpha blocker class of drugs.
