THE DIRECT COST COMPONENTS IN CHRONIC THROMBOEMBOLIC PULMONARY HYPERTENSION IN TURKEY- AN EXPERT PANEL APPROACH FOR ESTIMATION OF COSTS

OBJECTIVES: To estimate the direct cost components of chronic thromboembolic pulmonary hypertension (CTEPH) in a Turkish setting. METHODS: Delphi technique was used. An expert panel consisting of physicians from cardiology, pulmonology and cardiovascular surgery met at to discuss the disease management processes. The global and local literature and guidelines have been reviewed; questionnaires have been completed recording local clinical practices for each functional class (FC). All cost components, including medications, surgical treatment, hospitalization, screening and outpatient follow-up procedures were reviewed. Prices for February 2016 released by Ministry of Health and Social Security Institution in Turkey were used as references. February-2016 currency rate was used. RESULTS: The total costs, excluding pharmacologic treatment, of CTEPH/year for patients in FC-II, FC-III and FC-IV were calculated as 498€, 904€ and 5,466€, respectively. Since panel experts noted that disease-specific drugs are given only in FC-III, but are withheld or not started in FC-IV, the cost of pharmacologic treatment was set to be zero in FC-II and FC-IV patients, while it was 14,663€ in FC-III. Costs of other components, per year, used in calculating the total-cost in FC-II, FC-III and FC-IV were as follows: follow-up: 67€, 146€ and 1,355€; non-specific medications: 116€, 156€ and 1,867€; non-pharmacologic treatment: 126€, 366€ and 366€; laboratory tests: 188€, 236€ and 1,164€, respectively. Cost of lung transplantation was 714€ in FC-IV (2.5% of FC-IV patients were assumed to be transplanted and price of lung transplantation is 28,550€ [28,550x2.5%=714€])
