MEASUREMENT PROPERTIES OF THE EQ-5D-5L COMPARED TO THE EQ-5D-3L IN PSORIASIS PATIENTS

OBJECTIVES: To compare the measurement properties of the EQ-5D-3L and EQ-5D-5L in psoriasis patients in Hungary. METHODS: Outpatient and hospitalised patients with psoriasis in a single clinic completed both versions of the EQ-5D including EQ VAS and the Dermatology Life Quality Index (DLQI). The severity of psoriasis was assessed by Psoriasis Area and Severity Index (PASI). Both for 3L and 5L, the UK tariffs were applied to derive EQ-5D index scores. The 3L and 5L were compared in terms of missing values, redistribution properties, ceiling effects, informativity, convergent validity and known-groups validity. RESULTS: A total of 238 patients with psoriasis participated in the study. Overall, 30 unique health states were observed for 3L versus 86 for 5L. Mean EQ-5D-3L index scores were significantly lower compared to the 5L (0.77±0.26 and 0.84±0.19; p<0.001). There was only one missing value both on 5L and 3L suggesting an excellent feasibility. The overall ceiling effects decreased from 37.1 % (3L) to 32.9 % (5L). The proportion of inconsistent response pairs ranged from 1.3% (mobility) to 6.3% (anxiety/depression). Absolute informativity (Shannon’s index) improved significantly for most dimensions (5L 0.92-1.75 vs. 3L 0.59-1.22), but relative informativity (Shannon’s evenness index) improved only for three dimensions. A better convergent validity with PASI was confirmed in all dimensions, but only in mobility, usual activities and self-care dimensions with DLQI. The 3L indicated slightly better convergent validity with EQ VAS with the exception of the usual activities dimension. Known-groups validity was demonstrated both for 5L and 3L in terms of various clinical subtypes of psoriasis and treatment groups. CONCLUSIONS: This is the first study that compared the measurement properties of the EQ-5D-3L and EQ-5D-5L in a dermatological condition. The 5L version seems to improve measurement properties of the 3L in most aspects such as reducing ceiling effects and improving informativity.
