MANAGING EFFECTIVE ACCESS TO BIOSIMILARS- IN SEARCH OF AN OPTIMAL FRAMEWORK FOR CATALONIA

OBJECTIVES: To explore initiatives, policies and management tools necessary for the effective introduction and management of biosimilar drugs in Catalonia. METHODS:  A systematic review of European definitions, regulations, procedures and national experiences in the management of biosimilars was conducted and complemented by personal interviews with Catalan multi-disciplinary experts in the field. The research results were presented and discussed with a panel of thirteen multi-disciplinary experts led by CatSalut, using Metaplan® methodology, in order to analyse the possibilities and opportunities of adaptation and implementation to the Catalan context and needs. RESULTS: The systematic review shows that in EU prescription indicators, linked to specific policies and incentives, are the main tool for managing access to biosimilar drug. According to the opinion of the panel participating in the Metaplan® session, the establishment of biosimilar indicators could be the cornerstone for enhancing the use of biosimilar drugs in Catalonia. A total of 9 different indicators were identified: 6 for efficiency, 1 for acceptability, 1 for adherence and 1 linked to cost. The efficiency indicators, prioritised by 45% of the participants, focused on: number of patients using biosimilars (32%), use in naïve patients (9%) or rate of responders (4.5%). The development of specific healthcare policies to extend the use of biosimilars would include key aspects such as: (i) Securing adequate training of all stakeholders involved, establishment of consensus positioning, incorporation of biosimilars into clinical practice guidelines; (ii) adapt current drug introduction and management processes and (iii) monitoring of healthcare outcomes provided by prescription indicators. CONCLUSIONS: Quality prescription indicators would be the most appropriated tools for the management of biosimilar drugs in Catalonia. The definition of specific policies providing an overarching guide for informed decision-making will be a key success factor for the optimal introduction and use of biosimilars in Catalonia.
