OBJECTIVES:  To analyze how the inclusion of new essentially similar medicines in the Positive Drug List in Bulgaria changed reference price per DDD (defined daily dose) and medicines utilization in the group of statins. METHODS:  It is a retrospective, observational and statistical analysis of the changes in reference price per DDD (the lowest one within the same INN-International nonproprietary name) for the period 2009 - 2013 year. The medicines utilization is calculated by the DDD/1000/inh/day = ((Sales data in mg/ DDD)/(N inhabitans*365)). Prices was gathered from the PDL and sales data from the Bulgarian Drug Agency. Statistical significance is tested with t-test analysis. RESULTS: Within the observed period 3 new essentially similar products were included with INN simvastatin, 1 with fluvastatin, 8 with atorvastatin, and 10 with rosuvastatin. No new products were observed in the group of INN lovastatin. The reference price per DDD decreases statistically significant for all INNs between 3% and 86 % within observed period. The most significant change was observed for rosuvastatin, where were included high number of new products. The utilization expressed as DDD/1000inh/day increases between 2009 and 2013 years for Simvastatin (4.5 vs. 6.93), Atorvastatin (3.36 vs. 8.31), and Rosuvastatin (1.04 vs. 9.11). All changes are statistically significant. CONCLUSIONS:  Generic competition benefits both – the prices and utilization of essentially similar medicines. With the inclusion of new products within the groups with the same INNs the reference price per DDD is decreasing while the utilization in DDD/1000inh/day is increasing. This means that the patients’ access is improved.
