COST-EFFECTIVENESS ANALYSIS FOR TREATMENT OF PARKINSON’S DISEASE- A SYSTEMATIC REVIEW

OBJECTIVES:  In Japan, prevalence rate of Parkinson’s disease (PD) is reported around 100-150 per 100,000. Although a lack of resources in conducting cost-effectiveness analysis (CEA) has been pointed out, given that a pilot introduction of cost-effectiveness assessment has started since April 2016 in Japan, pharmaceutical and medical device industries need to establish practical approaches utilizing best available resources. The objective of this study was to systematically review cost-effectiveness studies of PD and evaluate the applicability of their analytical approaches to Japanese environment. METHODS:  Literature was searched from MEDLINE and Ichushi-Web based on the following criteria: 1) target disease is PD, 2) CEA, 3) published in the past 10 years, 4) English or Japanese (search date: March 18, 2016). Studies fulfilling the above criteria were further assessed on: country, model structure and simulation method, time horizon, perspective, source of parameters, and results. RESULTS:  18 studies met inclusion criteria, and most of the studies adopted Markov model (10 studies). Disease progression in the model was scored by Hoehn & Yahr scale (H&Y) and/or the changes in proportion of patients OFF-time during a day; five studies considered both, two considered only H&Y scale, two considered only OFF-time. The data for the transition probabilities were retrieved from literature or collected alongside clinical trials. Main sources of treatment related costs were claims data (6 studies), literature (7 studies), and clinical trial (5 studies). QOL scores were collected in parallel with clinical trials (7 studies) or from other QOL studies (11 studies). CONCLUSIONS:  Systematic review revealed that the data collection methods in prior studies were considered applicable to CEA for PD treatment in Japan. Although, QOL scores used in the model were referred to other QOL studies, they tend to be influenced by ethnicity. Further reports targeted on Japanese patients will be expected.
