COMPARISON OF THREE GENERIC UTILITY INSTRUMENTS IN PATIENTS WITH RENAL DISEASE

OBJECTIVES: The aim of this work is to build representative correction standards for the most widely used generic Health Related Quality of Life (HRQoL) questionnaires. METHODS: A prospective sample of patients, suffering from kidney diseases, was recruited by five researchers. The following instruments were autocompleted. HRQoL: Medical Survey Outcomes SF-36, EuroQoL (EQ-5D), Mark Health Utility Index-III (HUI). Disease and symptoms: Kidney symptom severity (FKSI-DRS), State-Trait Anger Expression Inventory (STAXI), Hospital Anxiety-Depression Scale (HADS). Sample population descriptive statistics and correlations were computed. All patients were under treatment for their health condition and gave their informed consent to participate in the study. RESULTS: An initial sample of 76 patients [mean age 52 years old (SD=16.2), 43.4% being women] was enrolled. Forty four percent suffered from congenital kidney disease and 56% from acquired renal disease. Mean disease span was 13.7 years (SD=13.79); 19.7% were following conservative treatment, 48.7% dialysis and 31.6% transplantation. Average kidney symptom severity (FKSI) was 8.3 points (SD=7.12), 41% could be diagnosed of clinical anxiety and 33% of clinical depression (HADS). Mean trait anger score (STAXI) was 19.94 (SD=7.17) and state 21.95 (SD=11.49). Mean score in SF-36 physical component was 35.6 (SD=7.002) and 38.2 (SD=14.74) in the mental component. Mean HRQoL VAS score was 57.4 (SD=22.20). Mean utility values were SF-6D=0.432 (SD=0.308), EQ-5D=0.682 (SD=0.215), and HUI=0.605 (SD=0.361). Utility distribution functions were found to differ notably in shape. CONCLUSIONS: Initial results show that generic HRQoL instruments are capable of capturing health deterioration in patients suffering from kidney disease, although specific instruments should be more suitable for diagnosis purposes. Meaningful differences were found between the three utility instruments, which should be taken into account when assessing or comparing groups of patients. These are preliminar results and a wider sample is to be gathered.
