OBJECTIVES: The aim of this study was to estimate the near-misses to prevent medication error reporting by Emergency Department (ED) Pharmacist in a tertiary care hospital Karachi, Pakistan.

METHODS: Retrospective chart review was done on near misses reported by ED pharmacist in a tertiary care hospital at Aga Khan University Hospital. These interventions were documented by ED pharmacists from the period of January 2019 till December 2019, which were then analyses on Microsoft excel and categorized for potential of harm if not intervened.

RESULTS: A total of 3205 pharmacist interventions are reviewed that was accepted during the study period. About 92% (n=2947) of the interventions were found to be moderately to very significant. Significant interventions were mostly belong to age group >60 year of age 27.5% (n=593). Most of intervention type related to wrong drug/mnemonic picked by physician for medication order entry 12 %( n=396) followed by incomplete/ unclear order 10 %( n=307). Data analysis showed that highest number of interventions belong to drug category of antibiotics followed by PPI/ H2 –antagonist/GI protective agents 19%(n=619).

CONCLUSIONS: Our study shows that ED Pharmacist plays pivotal role in reducing preventable medication errors which ultimately impact on decrease health care cost and patient hospital length of stay.
