Understanding the Real-World Evidence Published on FDA-Approved Immunotherapies in Oncology in the United States — a Targeted Literature Review

Objective: As immunotherapies are becoming standard for several cancers, an increasing amount of real-world evidence (RWE) is being generated to understand the wholistic view of these therapies. The aim of this targeted literature review was to understand and summarize key characteristics of the RWE published on FDA-approved immunotherapies across 17 different cancers. Methodology: Cochrane’s Rapid Review methodology was followed with an exhaustive search using PubMed, EMBASE, and Cochrane databases 2011 onwards (approval of first immunotherapy) for United States (US). Grey literature searches included conference proceedings from various oncology-specific conferences from 2019 to 2021, in line with the accepted methodology. Key information derived from the articles and abstracts included publication details, data sources, and the type of outcomes – clinical (e.g. survival), economic (e.g. cost and resource use), epidemiologic (e.g. incidence), and humanistic (e.g. quality of life). Results: 1,159 studies were included in the final analysis (334 journal articles; 825 conference abstracts). 62% of the studies used electronic medical records (EMR), followed by chart reviews (8%), registries (8%), claims (6%), primary data collection (6%). Very few studies combined various data sources (1%); with 7% not reporting the source. Overall, 92% of the studies focused on clinical outcomes, 5% on economic, 1% on humanistic, and 2% on any combination thereof. Almost all the studies utilizing EMRs and chart reviews studied clinical outcomes including treatment utilization, whereas about 50% studies utilizing claims data studied economic outcomes. Discussion: Some articles and conference abstracts didn’t mention data source (7%) and hence we couldn’t draw any insights from them. Currently, there is no single database that provides an insight into the clinical, economic, humanistic and epidemiology outcomes for oncology. There exists a need to harmonize data collection efforts across various entities to provide a complete picture of the patient journey and demands on the healthcare system.
