Effectiveness in Reducing the Risk of Death and Severity of a SET of Measures Against COVID-19 : The Case of an Insured Population in the Colombian Caribbean

OBJECTIVES

:

To estimate the effectiveness of a set of interventions against the severity and mortality due COVID-19 infection in a population from the Caribbean region of Colombia.

METHODS

:

A retrospective cohort study was conducted from a 59.424 confirmed and suspect COVID-19 cases affiliated in health insurance company between January and June 2020 in a population from the Caribbean region of Colombia. We used administrative database of all patients and made a follow-up time-person cohort with characteristics at baseline. The set of interventions includes monitoring, medical home or virtual care, with oximetry, oxygen therapy and laboratory. The main outcomes were reduction in the risk severity watched in hospital admission and death. A multivariate Cox regression model was used to evaluate the association between exposure to interventions and the outcomes. Hazard ratios and 95% confidence intervals were reported.

RESULTS

:

A total of 3,757 patients were included. Of them 1,877 were male with a mean of 42,7 years (SD 20.6); 498(13.3%) had comorbidity and 1,306(34.8%) had signs of infection at the beginning and 1,834 (48.8%) patients had at least some intervention. The mean time of follow-up was 15.3 days (SD 14.5); 727(19.4%) patients was recovery and 317(8.4%) deaths. For the patients had at least some intervention reduced their risk of severity at 60% (HR : 0.632; IC95% 0.542 – 0.736) and mortality at 83% (HR: 0.162 IC95% 0.123 – 0.214). Finally, for the risk of severity and mortality is high for symptomatics (HR: 3.671 IC95% 3.026 - 4.454 and HR : 1.834 IC95% 1.417 – 2.374) and patients with comorbidity (HR : 1.816 IC95% 3,026 – 4,454 and HR : 2,210 IC95% 1,752 – 2,786).

CONCLUSIONS

:

Preventive strategy intervention and closely follow-up of patients with COVID-19 can be an effective strategy for reducing the risk of severity and death approximately 60% and 83%.
