Association between Substance Use Disorder and Mental Health Service Utilization Among Adults with Major Depressive Disorder Using the National Survey on Drug Use and Health

OBJECTIVES: This study aimed to identify patient characteristics and examine the association between substance use disorder (SUD) and mental health-related healthcare resource utilization (HRU) among adults with major depressive disorder (MDD). METHODS: This retrospective longitudinal study used data from the 2015-2019 National Survey on Drug Use and Health (NSDUH), an annual nationally representative survey. Eligible individuals were ≥18 years of age, reported at least one major depressive episode in the past 12-month. Respondents reporting MDD vs both MDD and SUD were compared with respect to sociodemographic characteristics, health status, and mental health-related HRU. Multivariable logistic regression models were conducted to assess the mental health-related HRU between cohorts. RESULTS: Overall, 4,918 participants were included in the MDD+SUD cohort, and 14,612 participants were included in the MDD alone cohort. Compared to the MDD alone cohort, the MDD+SUD cohort was more likely to be younger, male, single, living in the large metro area, unemployed, and to have suicidal ideation and smoking habit. Within the MDD+SUD cohort, 54.6% of them reported mental health service use. Adults with co-occurring MDD and SUD were found to be more likely to report mental health-related HRU compared to their counterparts (any visit: adjusted odds ratio [aOR]=1.26, p<.001; inpatient visit: aOR=2.19, p<.001). The female, non-Hispanic white, insured, and unemployed respondents were more likely to report mental health-related service use among the co-occurring MDD and SUD cohort. CONCLUSIONS: Findings demonstrate the differences in patient characteristics and underscore the higher rates of mental health-related HRU among adults with co-occurring MDD and SUD compared to adults with only MDD.
