Assessment of Undiagnosed Hypertension in Peru: Epidemiology and Socioeconomic Inequalities, 2019

OBJECTIVES. To determine the presence of epidemiological factors and socioeconomic inequalities in undiagnosed hypertension in Peru. METHODS. We conducted an observational, cross-sectional, and analytical study using the Peruvian Demographic Health Survey (ENDES) of the year 2019. The dependent variable was the presence of undiagnosed hypertension (systolic blood pressure values ≥ 140 mmHg and/or diastolic blood pressure ≥ 90 mmHg in a second blood pressure measurement and in addition, no health personnel had previously informed that the patient had hypertension). Adjusted prevalence ratios (aPR) were estimated to identify the factors associated with undiagnosed hypertension. Socioeconomic inequality in undiagnosed hypertension was estimated using concentration curve (CC) and Erreygers concentration indices (ECI). RESULTS. Among the 3,600 patients with hypertension, the majority remained undiagnosed (66.7%). Undiagnosed hypertension was more prevalent in men (aPR = 1.16; 95%CI: 1.06-1.27), residents of the Coastal region (aPR = 1.13; 95%CI: 1.04-1.24), and in inhabitants over 3000 mASL (aPR =1.21; 95%CI: 1.02-1.42). Being in the age groups 50 to 69 (aPR = 0.80; 95%CI: 0.73-0.88) and 70 and over (aPR = 0.58; 95%CI: 0.50-0.68), having health insurance (aPR = 0.93; 95%CI: 0.87-0.99), being obese (aPR = 0.84; 95%CI: 0.76-0.94), and having diabetes mellitus (aPR = 0.55; 95%CI: 0.43-0.71) were associated with a lower prevalence of undiagnosed hypertension. An inequality was found in the presence of undiagnosed hypertension, concentrated in the poorest population (ECI = -0.0854). CONCLUSIONS. At least two out of three Peruvian adults with hypertension have no diagnosis of the disease. There are sociodemographic and health-related factors associated with, as well as socioeconomic inequality (concentrated in the poorest individuals), for undiagnosed hypertension.
