BUDGET IMPACT ANALYSIS OF SENSOR-AUGMENTED PUMP IN TYPE 1 DIABETES MELLITUS COMPARED WITH CONTINUOUS SUBCUTANEOUS INSULIN INFUSION IN THE RUSSIAN FEDERATION

OBJECTIVES : new and improved technologies for the treatment of diabetes continue to emerge at an impressive rate. Recent advancements in insulin delivery and continuous glucose monitoring (CGM) have made a particularly meaningful impact on the care and treatment burden of many people living with type 1 diabetes (T1D).The aim of research was to conduct budget impact analysis of sensor-augmented pump (SAP) compared continuous subcutaneous insulin infusion (CSII) in type 1 diabetes mellitus (T1DM) in the Russian Federation.

METHODS : a budget impact model was developed in MS Excel to simulate the costs of SAP compared CSII in T1DM for 15 year time horizon. Direct costs included the cost of treatment the complications of T1DM, insulin pump, CGM sensor, transmitter, glucometer, test-strips, the ultrashort insulin, infusion sets and reservoirs. The cost of consumables takes into account the time of its use: insulin pump – 6 years, CGM senor, infusion sets and reservoirs – 6 days, cannula – 3 days, test-strips – 4 times a day. Indirect costs included: loss of gross domestic product due to premature mortality, disability pension. The number of patients with type 1 diabetes and HbA1c level ≥8.0% in the Russian Federation is 93 258. The exchange rate used $ 1 = 62 RUB.

RESULTS : in accordance with the results of the budget impact analysis 93 258 patients for 15 year time horizon using SAP leads in additional funding $ 5 700 895 655 per years. The major share of SAP treatment costs was the replacement of CGM sensors and amounted to $ 3 904 779. The SAP therapy population per year requires additional funding $ 380 059 710.

CONCLUSIONS : the results of the budget impact analysis showed that switching to SAP requires additional funding.
