THE SAFETY AND EFFICACY OF MINI MICRO PERCUTANEOUS NEPHROLITHOTOMY (MMP) IN THE MANAGEMENT OF RENAL CALCULI OF SIZE 1-2 CM

OBJECTIVES : Percutaneous Nephrolithotomy(PCNL) is usually performed through 24 to 30Fr tracts. Reports have suggested that the risk of bleeding is associated with Amplatz size. To reduce the morbidity related to larger tracts modifications to the size of the instruments have been made. Mini Micro Pcnl(MMP) using 7.5Fr nephroscope has been developed for the treatment of renal calculi 1-2cm.To study the safety and Efficacy of MMP; the Operative time, hospital stay, pain score and analgesic requirement METHODS : We prospectively evaluated the outcomes of MMP in a single surgeon/Centre study. 40 patients underwent MMP from May 2016 to Dec 2017. On 1st month and 3 rd month postoperatively they were assessed by CTKUB for stone free status. RESULTS : The mean age was 43.43 years. 29 were males and 11 were females. The Mean stone size was 15.13 mm. 22 patients had calculus size of 1.2 -1.5 cms, 18 patients had calculus size of 1.6- 2 cms. Access was through superior (19) middle (13) lower calyx (8) The Mean operative time was 40.48 mins. Mean haemoglobin loss was 0.663 gm/dl. Mean creatinine rise was 0.07 m/dl. Grade 1 Clavein Dindo complication i.e., fever (7) was present. Single dose parental analgesia was given in 82% patients. Mean hospital stay was 36.35 hours. Stone free rate was 90% at 1st month, 100% at 3rd month. CONCLUSIONS Mini-Micro PCNL is a safe and efficacious technique in the management of renal calculi of size < 2 cms. This procedure can be recommended as it gives excellent stone free status, reduced analgesic requirements, early post op recovery and is cost effective.
