LITERATURE REVIEW OF PATIENT-REPORTED OUTCOMES (PRO) ASSOCIATED WITH MULTIPLE DAILY INSULIN INJECTION (MDI) THERAPY FOR PATIENTS WITH TYPE 2 DIABETES MELLITUS (T2DM)

OBJECTIVES: To summarize PROs for patients with T2DM on MDI therapy. METHODS: A literature review of studies published between January 2008 and January 2018 was conducted using PubMed and conference proceedings from June 2016 - June 2018. Studies were limited to those published in English and conducted in patients with T2DM on MDI. Studies including inpatients, older persons, pregnant women, or patients with an artificial pancreas or T1DM, or studies comparing insulin formulations were excluded. RESULTS: Of 2832 studies found, 15 studies reported PROs. MDI patients demonstrated significantly worsened mental burden due to worries about hypoglycemia and self-injecting away from home. However, MDI patients also experienced significant improvement in diabetes-related quality of life (QoL) and physician and regimen-related distress. Patients who switched from basal insulin to MDI experienced significant improvement in health-related QoL. Patients who switched from MDI to continuous subcutaneous insulin infusion (CSII) demonstrated significant improvement in psychological well-being, hypoglycemia and hyperglycemia symptoms, total diabetes symptoms, and health-related QoL. Patients who continued on MDI therapy demonstrated significantly reduced treatment satisfaction. However, MDI patients with improved HbA1c demonstrated significantly higher treatment satisfaction compared to those with no improvement in HbA1c. Patients who switched from MDI to CSII reported improved treatment satisfaction and ease of use. CONCLUSIONS: The review of the literature revealed that patients with T2DM on MDI face psychological burden and variations in QoL and treatment satisfaction. While some patients experienced improvements in QoL after initiating and/or continuing MDI therapy, others experienced improved outcomes after switching to alternative insulin delivery devices. Further patient-centered research is needed for patients with T2DM on MDI therapy.
