IMPACT TO DIABETES QUALITY PERFORMANCE MEASURES IN AN OLDER ADULT POPULATION WHEN USING A 24-HOUR WEARABLE INSULIN DELIVERY DEVICE FOR BASAL-BOLUS INSULIN THERAPY

OBJECTIVES: The financial impact associated with complications from diabetes is well documented and patients with poor control (A1C >9%) are at higher risk for complications with an older population being more vulnerable. Quality Performance Measures (QPM) were established to improve the quality of care with blood sugar control being a key indicator for diabetes management. This evaluation measured the impact to QPM and other clinical outcomes following the initiation of a 24-hour disposable insulin delivery device (V-Go) for basal-bolus therapy in older adults inadequately controlled. METHODS: An electronic medical records database was queried to identify patients with type 2 diabetes exceeding glycemic targets and initiated on V-Go. Evaluated variables were extracted at baseline and for multiple visits post initiation. Paired t-tests were used to compare outcomes to baseline for change in A1C and TDD and McNemar’s tests were used to compare the % attainment of QPM at baseline to on V-Go. RESULTS: Evaluated patients (N=54) had a baseline mean ± SD age of 71 ± 4 years (range 65 to 83), A1C 9.5 ± 1.6% and TDD 64 ± 41 u/day with 81% of patients prescribed insulin and 83% prescribed concomitant anti-hyperglycemic agents. After a mean duration of 5 months using V-Go, significant improvements in attainment of blood sugar control QPM were observed across the population (A1C > 9% decreased from 59% to 22%; p<0.0001 and A1C < 8% increased from 20% to 52%; p<0.0001) with V-Go. Mean ± SD decreases in A1C (-1.5 ± 1.8%; p<0.0001) and TDD (-9 ± 35 u/day; p=0.107) were also realized. Prevalence of severe hypoglycemia was markedly decreased from 13% at baseline to 2% with V-Go. CONCLUSIONS: In an older diabetes population, initiation of V-Go resulted in better attainment of QPM and decreased the prevalence of hypoglycemia compared to prior therapy which could have positive financial implications.
