OBJECTIVES: Describe HRQOL by line of therapy (LOT) and progression status in mUC in France, Germany, Italy, and Spain (EU-4) and the United Kingdom.

METHODS: Data were collected (November 16, 2020–April 21, 2021) for adults with mUC via chart review using a validated method. Demographic/Clinical characteristics and HRQOL data were collected at a single time point for patients in various stages of treatment. Following physician submission of a patient-record form and consultation, patients were asked to voluntarily complete the EQ-5D-5L (range, 0 [death] to 1 [full health]) and European Organisation for Research and Treatment of Cancer (EORTC; global health [QL2] score; range, 0–100, with higher scores indicating higher functioning) quality-of-life questionnaires. HRQOL was assessed overall and by LOT, response, and previous treatments.

RESULTS: A total of 617 patients with mUC completed the questionnaires and were included in the study (mean age, 69 years; men, 76%; currently/formerly smoking, 81%; mean BMI, 24.8 kg/m²; retired, 74%; most recent Eastern Cooperative Oncology Group performance status 0–1, 65%). At time of survey, mean EQ-5D-5L index and EORTC QL2 scores were 0.78 and 52.8 (n=608/610), respectively. Mean EQ-5D-5L index and EORTC QL2 scores were 0.81 and 55.9 for patients receiving first-line (n=252/253), 0.78 and 53.4 for those receiving second-line (n=198/198), and 0.73 and 47.1 for those receiving third-line or later LOT (n=158/159), respectively. Mean EQ-5D-5L index and EORTC QL2 scores were 0.83 and 56.9 for patients responding to treatment (n=228/228) and 0.64 and 42.5 for patients with progression (n=95/95), respectively. Patients who received chemotherapy and a PD-1/L1 inhibitor prior to completing the questionnaires reported mean EQ-5D-5L index and EORTC QL2 scores of 0.72 and 45.0 (n=101/102), respectively.

CONCLUSIONS: HRQOL decreased with additional LOTs and with progression. These results highlight the humanistic burden of mUC and need for treatments that improve HRQOL.
