Platinum Free-Interval in Patients With Ovarian Cancer Across an Argentinian Database: OCEANIA Real-World Study

OBJECTIVES:

Disease recurrence is common in patients with ovarian cancer (OC), with many requiring multiple lines of therapy (LOTs). Platinum-based therapy remains the cornerstone of treatment for platinum-sensitive patients. The platinum-free interval (PFI) is an important prognostic factor for progression-free survival. The aim of this real-world study was to evaluate PFIs across multiple LOTs for Argentinian patients with OC.

METHODS:

OCEANIA, a retrospective, open-cohort study, evaluated treatment patterns and clinical outcomes using a database from a private healthcare provider (Hospital Italiano de Buenos Aires). Patients who had >2 OC-related medical code and/or OC-related procedure/treatments were included in the analysis. The index date (proxy for diagnosis) was taken as the patient’s first OC-related medical code and/or encounter. LOTs were identified based on drugs dispensed between 2010‒2019; first LOT was within the initial 30-day period, a gap of 120 days between consecutive dispensation dates defined a change in LOT. PFI was defined as the interval (months) between the last recorded dose of one platinum-based LOT (pLOT) and first dose of the next pLOT, calculated for up to 5 pLOTs.

RESULTS:

A total of 741 patients with OC were identified, with mean follow-up of 31.7 (standard deviation [SD]=30.6) months and median 23.3 months. Of these, 355 (47.9%) received antineoplastic therapy. Among these, 90.4% (n=321) underwent 1 line(L) of platinum-based chemotherapy. Mean PFI(n;SD) of platinum-based chemotherapy from 1L2L, 2L‒3L, 3L‒4L and 4L‒5L was 27.1(n=116;15.5), 20.1(n=38;9.3), 18.2(n=11;8.0) and 8.9(n=3;3.8) months, respectively.

CONCLUSIONS:

The OCEANIA study showed a decline in PFI across successive pLOTs in patients with OC from the Argentinian database, likely due to increased resistance to therapies, and/or decreased tolerability, which may limit future treatment options. Therapies that prolong time-to-progression and time-to-next-treatment will help improve outcomes for patients with OC.
