Phase Angle, Quality of Life and Survival in Women With Head and Neck Cancer: An Uncommon Condition

INTRODUCTION: The quality of life (QoL) of women with non-thyroid head and neck cancer (H&NC), is related to their nutritional status. In this report health-related quality of life, survival and risk of death and their relationship to phase angle (PA) were studied.

OBJECTIVES: To determine the role of phase angle in survival and QoL in a women population with H&N cancer

METHODS: The selected subjects were divided into two groups: women with PA < 4.30° and women with PA ≥ 4.30°. Both of them were followed up for at least two years. We used EORTC QLQ-C30 and H&N35 to evaluate QoL, and Kaplan-Meier to analyze survival rate.

RESULTS: The sample studied consisted of 32 patients with a mean age of 57.6 years. Patients with a PA ≥ 4.30 had an ≈50% higher probability of survival at two years with a better quality of life compared to patients with a PA< 4.47°. Statistical differences were found in the functional and symptom scales, with lower functional scores and higher symptom scores in patients with low PA. In QLQ-C30 the observed differences were Global Health State/QoL (mean 70.8 versus 75; p=0.254), physical (66.7 versus 84.4, p=0.05) and role (71.7 versus 76.4, p=0.711) functioning, and loss of appetite (43.3 versus 16.7; p=0.029), in QLQ H&N35: senses problems (33.2 versus 5.6; p=0.007), speech problems (41.1 versus 16.7; p=0.047), Teeth (56 versus 25; p=0.036), sticky saliva (52.1 versus 19.4, p=0.017). Median survival was 16.7 months for those with PA <4.3° versus 32.7 months for those with PA ≥ 4.3° (p<0.026). The percentage of deceased subjects 75% versus 33.3%; p=0.041).

CONCLUSIONS: Even with an extremely low number of female patients with non-thyroid H&N cancer, phase angle lower than 4.3º was the most crucial predictor of impaired QoL, worse survival and a risk factor for death.
