OBJECTIVES: the objective of this analysis was to evaluate the incremental cost-effectiveness of the addition of dapagliflozin to the standard treatment compared to the standard treatment in the treatment of HF with reduced ejection fraction from the perspective of the Colombian health system.

METHODS: A Markov model was adapted using a hypothetical cohort of 1000 patients which was developed using information from the DAPA-HF study. Health states were the quartiles of the KCCQ-TSS, and transitory health states were included to assess the incidence of consultations and hospitalizations for heart failure. The time horizon was 5 years, and a 5% discount rate was applied, both for costs and outcomes. The information on costs and utilities was obtained from published literature. The costs were expressed in Colombian pesos as of 2020 (1 USD = $3,693.36 COP).

RESULTS: In the overall analysis, the ICER of the intervention compared to the comparator was USD $5,946 per QALY gained. In the analysis by subgroups, the ICER was USD $4,881/QALY gained in those with concomitant DM2, USD $6,868 in non-diabetics, USD $6,542 in those under 65 years of age and USD $5,441 for those over 65 years of age. The calculated ICERs were below the threshold of 3 times the GDP per capita (USD $17,130) informally adopted in Colombia.

CONCLUSIONS: From the perspective of the analysis, the addition of dapagliflozin to standard treatment is a cost-effective option in patients with heart failure with reduced ejection fraction in Colombia. The intervention remains a cost-effective option within the different analysis subgroups, obtaining a better cost-effectiveness ratio in patients with DM2 as comorbidity.
