OBJECTIVES: There are fewer studies of utility values that focus on different health status specifically within metastatic pancreas cancer or specific adverse events and related to chemotherapy using vignette-based methods. We evaluated Japanese societal-based health related utility values for metastatic pancreas cancer and different grade toxicities commonly associated with chemotherapy treatments using composite time trade-off (cTTO). METHODS: We developed health status scenarios of patients with metastatic pancreatic cancer undergoing chemotherapy based on a literature review and exploratory interviews. These scenarios were confirmed in cognitive interviews with the general public and clinical experts to validate the content. Selected adverse events included neutropenia, febrile neutropenia, diarrhea, nausea and vomiting and neuropathy. Each AEs were classified Grade 1/2 and 3/4 based on Common Terminology Criteria for Adverse Events version. We evaluated health related utility values of eleven scenarios in a societal-based valuation study based on the face-to-face method. Participants rated these scenarios in the cTTO and the visual analog scale (VAS) of the EuroQol valuation technology (EQ-VT) to derive health state utility scores. RESULTS: 220 responders answered in this survey. The stable disease with no adverse events (our reference state) had a mean utility value of 0.653 by cTTO. Lowest mean utility scores in stable states were 0.242 (stable disease + grade 3/4 vomiting). VAS results ranged from 0.189 to 0.468 depending on the type and grades of side effects in the stable disease. This result was affected by the ceiling and floor effects but showed a similar trend to cTTO results. CONCLUSIONS: We clarified health state utility values based on the general population for metastatic pancreatic cancer in Japan.
