Activity-Based Costing for Aneurysm's Endovascular Repair in Italy

OBJECTIVES

In Italy, analytical cost data at the individual NHS hospitals level are insufficient to meet the requirements of any approach to determining costs for DRG. Italian healthcare system is mainly public financed and an old DRG version is in use and a periodic up-date of the relative weights is missing. The objective of this research was to determine costs of aneurysm endovascular repair according to the real events happening in a mid-volume hemodynamic cat lab in Como (ASST Lariana).

METHODS

All 2018 SDOs (Hospital Discharge forms) from the hemodynamic cath-lab were analyzed. Unit costs were allocated to each operating session by production factor/activity of the incurred costs. A dedicated session on material used during each intervention was designed ad hoc.

RESULTS

In total 941 SDOs were analyzed. The procedure codes for endovascular repair was found in 37 cases: 33 abdominal (39.71) and 4 thoracic (39.73). For the abdominal patients the average age was 75.9 years with a mean hospital length of stay of 6.4 days and 0.5 days ICU. The average hospital cost was € 17,242.86 for abdominal (73% only procedure related) and € 36,597.38 for thoracic (47% only procedure related).

CONCLUSIONS

Our template for the collection and the processing of analytical data offers an opportunity to experiment a shared model at local level, where the situation is characterized by a considerable heterogeneity of the available data, both in terms of content as well as quality.
