How Do the Utilities for Promis-29 and EQ-5D-5L Compare? a Comparison of Propr and EQ-5D-5L VALUE SETS, and Implications for Users.

Objectives The EQ-5D-5L and its value sets are widely used around the world. However, in the US and elsewhere, there is growing use of the PROMIS, which has a value set (PROPr) based on the stated preferences of the US population. Both EQ-5D-5L and PROMIS are potential candidates for use in economic evaluation. This paper aims to compare the characteristics of EQ-5D-5L value sets and the PROPr scoring system and to highlight potential implications for users. Methods US, Australia and England value sets were used for EQ-5D-5L. PROPr scores were calculated using the PROMIS 29+2 v2.1 instrument. We examine various characteristics and distribution of all the ‘theoretically’ possible utilities and utilities for ‘comparable states’ in EQ-5D-5L and PROPr, and utility decrements within the dimensions of each instrument. Results EQ-5D-5L and PROPr utilities differ substantially. First, the US 5L utilities range from -0.573 to 1, while PROPr, calculated using PROMIS-29+2, ranges from -0.022 to 0.954. The distribution of theoretical values of PROPr is skewed, and PROPr has lower values than EQ-5D-5L for many comparable states. Second, in the 5L value sets considered, pain is the most important dimension whereas in PROPr pain is the least important (apart from sleep disturbance). Third, in 5L value sets, the greatest changes in utility occurred between levels 3 and 4. In PROPr that occurred between ‘corner’ states and their adjacent states. Conclusion The differences between the EQ-5D-5L value sets and PROPr have implications for their use in estimating QALY gains and decision making. The smaller range of PROPr means estimates of QALYs gains might be smaller, although interventions that result in improvements in the most severe states may result in larger QALY gains, compared to 5L utilities. The characteristics of PROPr may be attributable to the specific methods used to obtain preferences.
