A COMPARATIVE STUDY ON CLINICAL PRESENTATION AND PROGNOSIS OF STROKE IN THE LEFT AND RIGHT CEREBRAL HEMISPHERES

OBJECTIVES: Understanding the mechanism underlying stroke can aid the development of the therapies and improve the final outcomes. To assess how the side of the brain on which the lesion appears affects the rate and amount of stroke recovery. To explore possible prognostic variables of functional recovery in stroke patients with hemispheric involvement which is the dominant side. To determine whether in adult patients there is a higher incidence. The purpose of the study is to compare the characteristic mechanistic differences in the severity, functional outcomes, clinical presentation and prognosis of stroke in the left and right cerebral hemisphere. The present study aims to describe the clinical presentation along with the prognosis of the stroke. METHODS: A hospital based retrospective concurrent observational study was conducted. A structured data collection form was used to collect all the necessary relevant information from 150 patients in accordance with varying types of stroke the patients were assessed and collected data was analysed, interpreted and represented graphically. The patients are subjected to medical examination which includes collecting history, clinical examination, routine laboratory investigations, NIHSS Scale and GCS. RESULTS:The results indicated that the 150 patients enrolled in our study were distributed into the Right Ischemic Stroke (RHS), Left Ischemic Stroke (LHS), Right hemorrhagic Stroke, Left hemorrhagic stroke groups based on their final diagnosis. Compared to RHS, LHS were more severe at presentation, with a higher median NIHSS score (LHS: 9 vs. RHS: 6; p value = 0.011) and lower admission median GCS score (LHS: 13.5 vs. RHS: 14; p value = 0.005). However, the median area of the lesion of right hemispheric stroke is larger than that of left hemispheric stroke. CONCLUSIONS:This study demonstrated that the left hemispheric stokes are more common, more severe with worse outcome and often have a poorer prognosis than the right hemispheric strokes.
