EXAMINATION OF FACTORS INFLUENCING THE OCCURRENCE OF THROMBOSIS IN MYELOPROLIFERATIVE NEOPLASMS (MPN) BASED ON HUNGARIAN NATIONAL REGISTRY DATA

OBJECTIVES : Several questions have remained unanswered regarding risk factors impacting upon thrombotic events in myeloproliferative neoplasms (MPN). The aim of our study was to examination of factors influencing the occurrence of thrombosis in myeloproliferative neoplasms (MPN) based on Hungarian National Registry Data.

METHODS : Data analysed were derived from the Hungarian Myeloproliferative Registry (MOMR) that included records from 15 Hungarian health care institutions (n=894).Retrospective analysis was performed between 16.01.2013-30.06.2015, Variables analysed included: sex, age, type of myeloproliferative neoplasm,Janus-Kinaz (JAK) status (positive, negative), monocyte count (normal, high), occurrence of thrombosis, type 2. diabetes in the history. Besides descriptive statistics, c2-test was performed (p<0.05) using SPSS software.

RESULTS : Mean age in the sample was 60±13.6 years, 44.4% were men, 53.4% were women, JAK2 positive status rate was 69.6%, JAK2 negative 21.5%. Thrombotic event occurred in 117 men and 110 women. The occurrence of thrombosis was significantly higher among men (33% vs. 26%; p=0.022). In JAK2 negative cases (69% vs. 77%; p=0.036) the thrombotic rate was higher, nevertheless, in men it was more frequently associated with JAK2 positive status (35.8%vs. 26.8%; p=0.023). High thrombocyte count was found to have been associated with a significantly higher thrombosis rate (13% vs. 7%; p=0.016). However, the concomitant occurrence of high monocyte count and type 2. diabetes did not result in a more frequent occurrence of thrombotic events (p=0.284).

CONCLUSIONS : There is a need to establish a prospective register representing the patient population and the number healthcare institutions in order to contribute to and facilitate decision-making at the level healthcare and policy-making as well. Cardiovascular events are common in MPN the prevention of which and that of CVD-related mortality is a pivotal public health concern. To improve both health gains and the gain sharing of healthcare, General Practitioners should become active participants in education.
