Review On:-Cardiovascular Risk Reduction With Icosapent Ethyl For Hypertriglyceridemia

OBJECTIVES:- TO REVIEW CARDIOVASCULAR RISK REDUCTION WITH ICOSAPENT ETHYL FOR HYPERTRIGLYCERIDEMIA

METHODS:- Patients with elevated triglyceride levels are at increased risk for ischemic events. Icosapent ethyl, a highly purified eicosapentaenoic acid ethyl ester, lowers triglyceride levels, but data are needed to determine its effects on ischemic events. VASCEPA has multiple effects that extend beyond TG lowering, including increased EPA lipid composition from carotid plaque specimens. Vascepa, was found to reduce the risk of cardiovascular disease in patients by 25%. Vascepa was approved by the Food and Drug Administration in 2012 to treat people with high levels of triglycerides, a type of fat found in blood. Icosapent ethyl 2 g twice daily helped in reducing TGs, CV events, and CV death among patients with high TGs and either known CV disease or those at high risk for developing it, and who were already on statin therapy with relatively well-controlled LDL levels. In addition to lowering triglycerides, icosapent ethyl is known to lower apoB levels, a marker for circulating atherogenic lipoproteins.

RESULTS:- Percent change in TG for VASCEPA 4 g/d compared to placebo was -19.7% and percent change in LDL-C for VASCEPA 4 g/d compared to placebo was -6.6%. It reduces cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, coronary revascularization, or unstable angina. The result was found much faster when a icosapent was used.Atherosclerotic cardiovascular disease (ASCVD) was reduced. It has been rigorously demonstrated to decrease residual risk for total ischemic events in patients with ASCVD or with T2DM and other risk factors who had been receiving statin therapy

CONCLUSIONS:- The risk was significantly lower, by 25%. Rates of revascularization and MI were lower, while atrial fibrillation/flutter and bleeding were higher with icosapent ethyl.Hazard ratios was 95% with other drugs. Cosapent ethyl was rigorously shown to decrease residual risk for cardiovascular events.
