Explore the Factors Associated with Experienced Hypersensitivity Reactions Among Cancer Patients Receiving Oxaliplatin - a Retrospective Study in a Medical Center in Taiwan

OBJECTIVES: Hypersensitivity reactions (HSRs), e.g., rash, itchy, were the common adverse drug reactions during receiving chemotherapy (i.e., Oxaliplatin). Due to lack of potential factors associated with HSR, the management strategies tend to treat HSRs after happening than prevention. This study aims to preliminarily explore the factors associated with experienced HSR with oxaliplatin in order to come up with better prevention strategies afterward. METHODS: Those patients receiving oxaliplatin and had occurred HSR in outpatient chemotherapy unit were grouped into either one-episode group or multi-episode group. Their following data from electronic medical chart or nurse records at the first HSR events were retrieved and compared between groups: demographic information, cancer type, medication usage, severity of HSR. We classified all enrolled patients into two groups based on the number of HSR events. Patients who had 2 or more than 2 times HSR events were in group 1, and who had once HSR event were in group 2. The descriptive and inferential analyses using t tests or chi-square tests were performed to explore the difference between two groups. RESULTS: Of 138 patients experienced HSR from October of 2015 to 2019, 54.3% were continuously given oxaliplatin after occurring the first episode of HSR. Those patients in multi-episode group tended to be younger and less likely to use steroid as premedication than one-episode group (ps<0.05). There were no statistically significant differences in cancer region, severity of HSR and other characteristics between two groups. Among patients in one-episode group, 61.2% were discontinued oxaliplatin after occurring the first HSR while 68.3% of them were encountered severe HSR events. CONCLUSIONS: Age and prior use of steroids were associated with number of HSR episodes. It seems that premedication with steroids for younger cancer patients might prevent occurrence of oxaliplatin-related HSR but warrant further evaluation or research to confirm.
