Treatment Gaps in Deep Brain Stimulation for Patients with Parkinson's Disease: A Comparative Analysis of Nine High-Income Countries

OBJECTIVES: According to the literature, an estimated 4.5 - 10% of patients with PD are eligible for deep brain stimulation (DBS). However, health statistical data indicate significant differences in DBS utilization rates across countries. Reasons for differences vary between countries, including availability of, and reimbursement for, DBS surgery. This investigation aimed to determine current treatment gaps in DBS for Parkinson’s disease (PD) in nine high-income countries using DBS utilization rates, PD incidence, and estimated percentage of PD patients eligible for DBS surgery. METHODS: This policy analysis compared DBS utilization data in PD; patient eligibility for DBS; and PD incidence in Australia, Canada, Germany, Korea, New Zealand, Norway, Switzerland, UK and USA. Using the equation [(incidence of PD*eligible percentage of DBS patients) – actual utilization of DBS], national treatment gaps were calculated. The treatment gap was calculated using a one-way sensitivity analysis varying the eligibility percentage from 4.5% to 10%. RESULTS: Treatment gaps in DBS for PD were found across all countries analyzed. Assuming a 10% patient eligibility, and using 2017 PD incidence, New Zealand was found to have the largest treatment gap (2.2 patients/100,000/year; 2012 DBS utilisation data), followed by Canada (2.06/100,000/year; 2016 utilization data). The smallest treatment gaps were found in the USA (0.92/100,000/year; 2011 DBS utilization data) and Australia (1/100,000/year; 2013 DBS utilization data). CONCLUSIONS: The largest treatment gaps for DBS in PD were found in New Zealand and Canada; and the smallest in the USA and Australia. Treatment gaps seemed primarily driven by low DBS utilization rates combined with a high incidence of PD. By understanding the population age structure and political driving forces behind DBS treatment, access to care for PD patients can be improved.
