REAL-WORLD TREATMENT PATTERNS AMONG PATIENTS WITH FOLLICULAR LYMPHOMA IN KOREA: COMPARISONS BY POD24 STATUS USING NATIONWIDE CLAIMS DATA

OBJECTIVES: Follicular lymphoma (FL) is characterized by frequent relapse and heterogeneous treatment strategies. In particular, patients who experience disease progression within 24 months (POD24) after initial treatment are known to have a poor prognosis, yet treatment patterns and unmet needs in this high-risk population remain insufficiently characterized. This study aimed to compare real-world treatment patterns according to POD24 status among patients with FL in Korea.
METHODS: This retrospective cohort study used Korean nationwide claims data to identify adults newly diagnosed with FL between 2015 and 2019, with follow-up through July 2024. Patients initiating second-line treatment within 24 months of first-line treatment initiation were classified as the POD24 group, and those initiating later as the non-POD24 group. Treatment lines through third-line therapy were identified using an algorithm informed by Korean reimbursement criteria and clinical guidelines, and time to next treatment was assessed between successive lines.
RESULTS: Among 736 patients who initiated first-line treatment, 162 (22.0%) initiated second-line therapy; of these, 87 (53.7%) were classified as the POD24 group and 75 (46.3%) as the non-POD24 group. The most common second-line regimens were ICE (24.1%) and R-CHOP (17.2%) in the POD24 group, compared with rituximab monotherapy (22.7%) and obinutuzumab plus bendamustine (17.3%) in the non-POD24 group. Overall, 68 patients initiated third-line therapy, corresponding to 49.4% of the POD24 group and 33.3% of the non-POD24 group. The median time from initiation of second-line therapy to initiation of third-line therapy was shorter in the POD24 group than in the non-POD24 group (231 days vs 381 days).
CONCLUSIONS: Real-world treatment patterns differed according to POD24 status among patients with FL in Korea. Patients in the POD24 group showed more frequent progression to third-line therapy and a shorter time to subsequent treatment, highlighting the substantial treatment burden and unmet need for optimized treatment strategies in this high-risk population.
