INFLUENCING FACTORS OF PRICE REDUCTION FOR INNOVATIVE ANTICANCER DRUGS IN CHINA'S NATIONAL REIMBURSEMENT DRUG LIST NEGOTIATIONS FROM 2017 TO 2024

OBJECTIVES: This study aimed to systematically analyze the key factors associated with price reduction of innovative anticancer drugs during China’s national reimbursement drug list (NRDL) negotiations from 2017 to 2024, clarify the mechanisms of drug characteristics, clinical value, clinical trial design, and market environment on price reduction, and provide evidence-based support for optimizing negotiation strategies, balancing innovation incentives and patient access.
METHODS: We included 132 innovative anticancer drugs and 238 indications approved in China and successfully included in the NRDL through price negotiations between 2017 and 2024. Price reduction was calculated using monthly treatment costs before and after negotiations. Descriptive statistics, univariate and multiple linear regression models were applied to identify independent factors influencing price reduction.
RESULTS: The average annual price reduction of anticancer drugs during negotiations ranged from 44.2% to 65.2%, with a maximum reduction of 87% from 2017 to 2024. Univariate regression showed that price reduction was significantly associated with PFS improvement, QoL improvement, innovation type, orphan drug status, trial control type, global enrollment, marketing duration, and incidence rate.In multiple linear regression, innovation level (β=−8.97, P=0.015), orphan drug status (β=−9.83, P=0.004), PFS (β=−0.73, P=0.022), and global patient enrollment (β=−0.01, P=0.003) were significantly negatively associated with price reduction. In contrast, active control (β=10.79, P=0.008) and marketing duration (β=0.01, P=0.018) were significantly positively associated with price reduction.
CONCLUSIONS: Price reduction of anticancer drugs in China’s NRDL negotiations is jointly determined by clinical value, innovation attributes, clinical trial evidence, and marketing duration. Drugs with superior clinical benefits and higher innovation levels receive smaller price reductions, reflecting a value-oriented negotiation policy.
