COST-EFFECTIVENESS ASSESSMENT OF 20-VALENT PNEUMOCOCCAL CONJUGATE VACCINE (PCV20) AMONG OLDER ADULTS IN THE PHILIPPINES

OBJECTIVES: Publicly funded pneumococcal vaccination for older adults in Philippines comprises two doses of 23-valent pneumococcal polysaccharide vaccine (PPV23; PPV23+PPV23), whereas sequential vaccination with 13-valent pneumococcal conjugate vaccine (PCV13) and PPV23 (PCV13→PPV23) is used in private healthcare settings. We evaluated the cost-effectiveness of single-dose 20-valent PCV (PCV20) versus PPV23+PPV23 in adults aged ≥60 years from the government perspective and PCV13→PPV23 in adults aged ≥50 years from the private perspective.
METHODS: A Markov-cohort model projected lifetime cases, deaths, and costs associated with invasive pneumococcal disease (IPD) and non-bacteremic pneumococcal pneumonia (NBPP). Philippines-specific epidemiologic and cost inputs were employed where available. Vaccine uptake at model entry was 52.9% in government settings and 17.4% in private healthcare settings, with 100% completion of the subsequent dose (PPV23) in multi-dose strategies. Cost-effectiveness (cost per quality‑adjusted life‑year [QALY] gained) was assessed using willingness-to-pay (WTP) threshold of 1XPhilippine gross domestic product per capita (PHP230,494; discounting=7%/year). Probabilistic sensitivity analyses (1,000 simulations) were performed.
RESULTS: In both perspectives, PCV20 was dominant (i.e., less costly and more effective) compared to existing strategies. From the government perspective among adults aged ≥60 years, PCV20 prevented 10,354 cases of IPD and NBPP and 1,551 deaths, yielded 3,703 QALYs, and saved PHP175 million. From the private perspective among adults aged ≥50 years, PCV20 prevented 3,639 cases of IPD and NBPP and 514 deaths, yielded 1,673 QALYs, and saved PHP2,421 million. In probabilistic sensitivity analysis, PCV20 was dominant in 100% of simulations from both government and private payer perspectives.
CONCLUSIONS: Replacing current pneumococcal vaccination strategies for older adults in the Philippines with a single dose of PCV20 would improve health outcomes while reducing total healthcare costs. PCV20 offers programmatic advantages and may potentially encourage higher vaccine uptake.
