CLINICAL AND DEMOGRAPHIC CHARACTERISTICS OF INDIVIDUALS WITH INFERTILITY IN SPAIN: A DESCRIPTIVE STUDY FROM THE TELOTRON DATABASE

OBJECTIVES: In Spain, evidence on infertility has mainly relied on assisted reproductive technology registries, which provide information on treatment cycles and outcomes but offer limited insight into the clinical characteristics of individuals at the time of infertility diagnosis in routine clinical practice. Objectives: To characterize patients diagnosed with infertility according to sex, age, smoking status, comorbidities and medication use before and at infertility diagnosis date.
METHODS: We performed a retrospective population-based observational study using Telotrón®, a longitudinal healthcare database including over 2.2 million individuals from seven Spanish Autonomous Communities. Individuals aged 18-45 years with a first recorded diagnosis of infertility (ICD-9-CM 606/628; ICD-10-CM N46/N97) between 2018 and 2025 were included. Baseline demographic characteristics, lifestyle factors, comorbidities, fertility assessments, contraceptive use and concomitant medications were described. Comorbidities and treatments were identified using ICD and ATC codes, respectively. Annual incidence rates were estimated.
RESULTS: 9484 individuals were identified, with over 75% being female. The average age was 34.6 years (5.1), and only 3.6% of diagnoses occurred within the 18-25 age group. About 31.4% were current or former smokers, and the mean BMI was 25.9 (6.1). 66.9% of the population had at least one comorbidity related to the reproductive system, and 40% of women had menstrual alterations. The most common comorbidities in the cohort included anxiety/depression, thyroid disorders, dyslipidemia, and overweight/obesity. One-third of the cohort had undergone fertility testing, and both contraceptive medication use and history were noted at the index date. Additionally, medications were evaluated before or at the index date. Incidence rates remained stable over the study period.
CONCLUSIONS: This study provides one of the first real-world descriptions of individuals diagnosed with infertility within the Spanish public healthcare system. These findings provide a foundation for future real-world studies evaluating infertility care and outcomes in Spain.
