SOCIOECONOMIC BURDEN AND SOCIETAL VALUE OF INNOVATIVE DMT IN THE TREATMENT OF MULTIPLE SCLEROSIS IN ALGERIA

OBJECTIVES: Multiple sclerosis (MS) is the leading cause of non-traumatic neurological disability in young adults. This study quantifies the full annual economic burden of MS and the social value generated by innovative disease-modifying therapy (DMT) in a cohort of diagnosed and treated patients.
METHODS: A prevalence-based cost-of-illness (COI) model was applied to 4,997 diagnosed and treated MS patients in Algeria. Direct and indirect costs were captured. The social value of innovative DMT was additionally estimated using a multi-component social impact model, decomposing economic effects into paid direct, unpaid direct (informal care), paid indirect, and paid induced cost domains from the Wifor report, expressed in USD per patient.
RESULTS: Annual direct medical cost per patient varied substantially by disease subtype: USD 3,099 (Low Activity RRMS), USD 18,151 (Aggressive RRMS; 5.9-fold higher than Low Activity RRMS), and USD 1,756 (Active SPMS/PPMS). Indirect absenteeism costs were highest in SPMS/PPMS at USD 152 per patient. The total annual national direct medical burden was USD 14.1 million (DZD 1.89 billion), with total societal burden reaching USD 14.3 million (DZD 1.91 billion). Innovative DMT generated a social value of USD 876 per treated patient, comprising informal care cost offsets (USD 503; 57%), paid direct effects (USD 241; 27%), indirect productivity gains (USD 95; 11%), and induced economic effects (USD 37; 4%).
CONCLUSIONS: MS imposes a substantial and subtype-differentiated economic burden in Algeria, with Aggressive RRMS patients incurring direct costs 5.9-fold higher than Low Activity RRMS patients. Indirect productivity losses add USD 900 per treated patient across all subtypes. Innovative DMT generates USD 876 in additional social value per patient treated, primarily through informal care cost offsets. Together, these COI and social impact findings provide a comprehensive economic case for timely and equitable access to effective DMTs within Algerian health policy and reimbursement frameworks.
