HEALTHCARE UTILIZATION OF HEAD AND NECK CANCERS IN BRAZIL: REGIONAL AND AGE-SPECIFIC PATTERNS IN 2023

OBJECTIVES: To describe the outpatient and inpatient healthcare utilization patterns for head and neck cancers (HNC) in Brazil in 2023.
METHODS: A retrospective observational study was conducted using secondary data on HNC (subsites: Oropharyngeal, Laryngeal and Oral cavity) from the Brazilian public healthcare system, including outpatient information system (SIA) and inpatient information system (SIH) databases. Data were identified through ICD-10 codes and analyzed both in aggregate and by subsite, with stratification by sex, age group and geographic region. Crude rates per 100,000 population and age-standardized rates (ASRs), using the WHO World Standard Population, were calculated.
RESULTS: In 2023, HNC accounted for 101,276 outpatient consultations (crude rate 103.3/100,000; ASR 45.5/100,000) and 27,015 hospitalizations (crude rate 27.6/100,000; ASR 11.7/100,000) in Brazil. Men accounted for the largest proportion (75.8% of outpatient visits; 79.7% of hospitalizations). By subsite, oropharyngeal cancer was the leading contributor to outpatient care (40.6% of HNC), followed by laryngeal (30.8%) and oral cavity cancers (28.6%), whereas inpatient burden was highest for laryngeal cancer (45.3%), followed by oropharyngeal (31.4%) and oral cavity (23.3%). Healthcare utilization was distributed across all age groups with burden progressively increasing with age, with individuals aged >60 years accounted for 55.0% of outpatient consultations and 58.6% of hospitalizations. Outpatient ASRs were highest in the Southeast, led by Minas Gerais (77.6) and Espírito Santo (65.2), and in the North, represented by Amazonas (74.5); inpatient ASRs were highest in Rio Grande do Norte in the Northeast (26.7) and in Espírito Santo (19.2) and Minas Gerais (16.9) in the Southeast.
CONCLUSIONS: Healthcare utilization for HNC in Brazil shows substantial variation by sex, age, and region. The predominance of oropharyngeal cancers in outpatient care and laryngeal cancers in inpatient settings, together with the concentration of burden in older adults and specific regions, highlights important epidemiological patterns.
