HEALTHCARE RESOURCE UTILIZATION ASSOCIATED WITH PEDIATRIC PNEUMOCOCCAL DISEASE HOSPITALIZATIONS IN SPAIN: RESULTS FROM THE NATIONAL HOSPITAL DISCHARGE DATABASE

OBJECTIVES: To evaluate trends in invasive pneumococcal disease (IPD) and all-cause pneumonia (ACP) among children in Spain, and associated healthcare resource use (HCRU).
METHODS: Children hospitalized with IPD or ACP from 2016 to 2023 were identified using ICD-10 codes in the national hospital discharge database (RAE-CMBD). Four age groups were defined: infants (<12 months), toddlers (12-23 months), younger children (2-5 years), and older children (6-18 years). Outcomes included incidence rates (IRs; per 100,000 children), hospital length of stay (LoS; days), ICU admission (%), ICU length of stay (ICU LoS; days), and mean hospitalization costs (€), including inpatient and ICU care. Group differences are presented descriptively.
RESULTS: A total of 1,832 IPD (IR: 2.8) and 87,405 ACP (IR: 132.7) hospitalizations were identified during the study period. IPD IR was highest among infants (16.0 vs. 2.8 overall) and decreased in older age groups. Overall, IPD IR declined in 2020 before increasing across all age groups. Overall ACP incidence also declined in 2020. ACP IR was highest among infants and toddlers prior to 2020 (315.0 and 302.0) with a greater increase among toddlers in 2023 (538.1 and 763.8). ICU admission rates for IPD were similar across age groups (mean=30.7%). In contrast, ICU admission for ACP was highest among infants and older children (11.7% and 10.1% vs. 7.7% overall). For IPD, compared to the overall cohort, infants had longer stays (LoS: 13.8 vs. 12.6 days; ICU LoS: 3.3 vs. 2.4 days) and higher costs (€9.6K vs €8.3K). For ACP, infants also experienced longer stays (LoS: 10.0 vs. 6.5 days; ICU LoS: 2.0 vs. 1.2 days) and higher costs (€5.6K vs. €4.2K).
CONCLUSIONS: Infants bear the greatest burden of pediatric pneumococcal disease in Spain, with higher incidence, longer hospital stays, and greater costs. These findings support prioritization of infants through routine vaccination.
