BURDEN OF DISEASE OF COPD GOLD 2-3 IN LATIN AMERICA: A MODEL-BASED ASSESSMENT OF MILD, MODERATE, AND SEVERE EXACERBATIONS

OBJECTIVES: To estimate the burden of disease associated with Chronic obstructive pulmonary disease (COPD) GOLD 2-3 (2007) in patients over the age of 40 in Latin America, explicitly quantifying the contribution of mild, moderate, and severe exacerbations to cases, years lived with disability (YLD), years of life lost (YLL), and disability-adjusted life years (DALYs) in Argentina, Colombia, and Mexico.
METHODS: A deterministic burden-of-disease model was developed using published epidemiologic and clinical inputs, calibrating severity-specific parameters for Latin America. The model incorporated country-specific GOLD stage distributions alongside its mortality, and probabilities of mild, moderate, and severe exacerbations.
RESULTS: In GOLD 2, total YLDs were 88,705 in Argentina, 115,067 in Colombia, and 153,489 in Mexico. Exacerbation-related YLDs in GOLD 2 were driven mainly by moderate exacerbations—33,175, 43,034, and 57,404, respectively—followed by severe exacerbations (14,551, 18,875, and 25,178) and mild exacerbations (1,628, 2,112, and 2,817). Corresponding YLLs were 65,500, 58,075, and 120,667, yielding 154,205, 173,142, and 274,157 DALYs, respectively. In GOLD 3, total YLDs increased to 129,525 in Argentina, 123,261 in Colombia, and 303,923 in Mexico. Exacerbation-related YLDs remained concentrated in moderate exacerbations—75,392, 71,746, and 176,904, respectively—followed by severe exacerbations (34,452, 32,786, and 80,839) and mild exacerbations (510, 486, and 1,197). YLLs also rose substantially in GOLD 3, reaching 150,477 in Argentina, 133,419 in Colombia, and 277,215 in Mexico, resulting in 280,002, 256,680, and 581,138 DALYs, respectively. Overall, GOLD 3 showed higher YLDs, YLLs, and DALYs than GOLD 2 in all countries, with Mexico showing the highest absolute burden.
CONCLUSIONS: COPD GOLD 2-3 imposes a high clinical burden in Latin America. Explicit inclusion of mild, moderate, and severe exacerbations provides a more complete estimate of disease burden and shows that exacerbations are a key driver of disability, especially in older adults and in GOLD 3 Compared to GOLD 2 disease compared to GOLD 2.
