HEALTH-RELATED QUALITY OF LIFE IN HEART FAILURE IN SAUDI ARABIA: A COMPARATIVE ASSESSMENT USING THE EQ-5D-5L AND EQ-HWB-9

OBJECTIVES: To assess the convergent validity of the Arabic EQ Health and Wellbeing Short (EQ-HWB-9) with the Kingdom of Saudi Arabia (KSA) Arabic EQ-5D-5L in adult heart failure (HF) outpatients and explore associations between health-related quality of life (HRQoL) measures and patient characteristics.
METHODS: A cross-sectional study enrolled 69 adult HF outpatients attending King Khalid University Hospital (KKUH), Riyadh. Participants completed the KSA Arabic EQ-5D-5L, the Arabic EQ-HWB-9, and a demographic questionnaire. Spearman rank-order correlations (rs) were used to assess: (1) convergent validity between EQ-5D-5L/EQ VAS and EQ-HWB-9 items (54 pairs), and (2) associations between EQ-5D-5L domains and age, gender, self-rated health, and comorbidities. Descriptive statistics assessed HRQoL distributions.
RESULTS: Participants were predominantly aged 45-64 years (39.1%), male (56.7%) and Saudi nationals (87.3%). Hypertension (73.9%) and diabetes (69.6%) were the most common comorbidities. Strong convergent validity was observed between EQ-HWB-9 and EQ-5D-5L (46 of 54 domain pairs rs>0.50, all p<0.001). Usual Activities demonstrated the broadest convergence (rₛ = 0.536-0.876), strongest for HWB-Mobility (rₛ = 0.876) and HWB-Daily Activities (rₛ = 0.846). Self-Care correlated strongly across all EQ-HWB-9 items (rₛ = 0.566-0.839). EQ-VAS demonstrated strong negative convergence with functional wellbeing items (rₛ = −0.677 to −0.716; all p < 0.001). Self-rated health had the strongest associations with EQ-5D-5L domains (rs=0.50-0.85), followed by age group (rs up to 0.66 with Pain/Discomfort), while no significant associations were observed for gender.
CONCLUSIONS: The Arabic EQ-HWB-9 demonstrated strong correlation with the KSA Arabic EQ-5D-5L among heart failure outpatients, supporting its potential use for assessing health and wellbeing in Arabic-speaking populations.
