OBJECTIVES: HER2-positive breast cancer (HER2+ BC) disproportionately affects working-age women and imposes substantial socioeconomic costs through direct expenditure, productivity loss, and caregiving burden. This study quantifies the annual economic burden of HER2+ BC in Algeria and evaluates the social impact of innovative HER2-targeted therapies.
METHODS: A prevalence-based cost-of-illness model was built from patient-level data at an Algerian oncology institution. Epidemiological inputs drew from GLOBOCAN 5-year prevalence estimates and expert-validated stage distributions. Resource use across all treatment modalities was valued at nationally applicable unit costs; indirect costs were captured. The WifOR Social Impact methodology quantified disease burden via Disability-Adjusted Life Years (DALYs) and translated Quality-Adjusted Life Years (QALYs) from innovative therapies into gross value added (GVA) through direct, indirect, and induced economic effects.
RESULTS: An estimated 23,066 Algerians carry HER2+ BC; approximately 18,453 are treated annually, with Stage II (45.2%) and Stage III (33.6%) predominating. Total annual burden reaches USD 137.7 million (DZD 18.4 billion), with a mean per-patient cost of USD 7,462. Targeted therapy is the dominant cost driver at 54.1% of total expenditure, followed by radiotherapy at 32.5%; indirect costs represent 2.3%. Applying the WifOR methodology to Algeria-specific data, Pertuzumab+Trastuzumab-based regimens generated USD 1,541 per patient in social impact (559 patients; 39.4 QALYs) and T-DM1 generated USD 1,375 per patient (607 patients; 38 QALYs), yielding USD 1.7 million in HER2+-attributable social impact and 1,166 QALYs combined.
CONCLUSIONS: HER2+ BC imposes a substantial burden on Algeria's healthcare system and its working-age population. Innovative HER2-targeted therapies generate measurable returns exceeding treatment costs, providing a methodology-grounded evidence base to support value-based reimbursement and equitable access decisions in Algeria.
