EARLY ADOPTION OF THE ICHOM SET FOR OBESITY AND DEVELOPMENT OF A FEDERATED DATA-SHARING NETWORK TO SUPPORT INTERNATIONAL OUTCOMES BENCHMARKING

OBJECTIVES: The absence of standardised outcomes measurement in obesity care limits benchmarking, comparative effectiveness research and quality improvement across healthcare settings. To address this gap, ICHOM developed an outcome set for Obesity and launched an international learning collaborative to support implementation. This study evaluated early adoption of the Set and explored the feasibility of a federated data-sharing network to support multi-site outcomes comparisons.
METHODS: The Obesity Learning Collaborative includes multidisciplinary care teams from 13 healthcare organisations across 10 countries. Participating sites received implementation support through peer-learning sessions focused on patient-reported outcome measures, workflow integration, data collection, and use of outcomes data for service improvement. Adoption metrics included implementation status, mode of PROM collection, and readiness of data sharing. In parallel, two organisations participated in a pilot federated data-sharing network, whereby data remained under local control while standardised aggregated analyses were performed centrally.
RESULTS: As of June 2026, 13 organisations from 10 countries had joined the collaborative. Seven organisations (53.8%) had initiated routine collection of the Set and six (46%) had implemented electronic PROM collection. Common implementation challenges included PROM completion rates, patient retention, workflow integration and clinician engagement.
Among survey respondents, 100% reported that participation in the collaborative was beneficial, 53% reported improved understanding of outcomes measurement, 61% reported improved organisational alignment, and 30% reported implementing changes to their local approach based on shared learning.
Two organisations participate in the initial development and testing of a federated data-sharing network. Activities focus on data mapping, standardisation, governance processes and testing of federated analytical workflows. Evaluation of cross-site comparative analyses is ongoing.
CONCLUSIONS: Implementation of the Obesity Set is feasible across diverse international healthcare settings. Learning collaboratives may accelerate adoption through shared implementation experience, while federated data sharing approaches offer the potential to enable future international benchmarking and real-world outcomes research in obesity care.
