OBJECTIVES: To assess the six HTA pillars("6Ps") as an integrated framework for timely, equitable access from an organizational perspective.
METHODS: A multi-stage, mixed-methods consensus-generation study was conducted during ISPOR UAE 2026 with an expert panel (policymakers, payers, providers, industry, HTA experts). Using a modified nominal group technique, the process began with literature-informed identification of barriers, followed by structured, moderator-led discussions across the six pillars. Qualitative insights were generated through Cross-Impact Analysis to explore interdependencies, while real-time quantitative polling captured agreement. Findings were synthesized using thematic analysis, and consensus was predefined as ≥80% agreement.
RESULTS: Qualitative findings Cross-Impact Analysis confirms that the six pillars operate as an interdependent HTA governance system, with effectiveness achieved through integrated rather than standalone implementation.
CONCLUSIONS: The 6Ps form a coherent framework only when implemented as an integrated system. Phased integration offers a practical roadmap for emerging MENA systems

- Patient-Centered Value: Anchors HTA in patient-reported outcomes, quality of life, and preference-sensitive care
- Protected Integrity: Safeguards processes from conflicts of interest, undue influence, and capture by actors
- Proactive Access: Ensures HTA timelines align with regulatory pathways to prevent delays from impeding access
- Participatory Voices: Embeds multi-stakeholder engagement (patients, clinicians, payers, industry) across scoping, assessment, and deliberation
- Predictable Pathway: Establishes standardized submission, evaluation, and appeals procedures to reduce uncertainty for stakeholders
- Public Transparency: Requires disclosure of methods, evidence sources, rationales, and final decisions with justifications
- There was 80% consensus that the six HTA pillars ("6Ps") must be fully integrated within a reliability framework where value legitimacy, process predictability, transparency, and integrity collectively produce timely and equitable access.
