HUMAN PAPILLOMAVIRUS (HPV) VACCINATION COVERAGE AMONG FRENCH ADOLESCENTS AND CONTRIBUTION OF COMMUNITY PHARMACIST: PAPILLON STUDY

OBJECTIVES: The French cancer control strategy aims for 80% HPV vaccination coverage at 15 by 2030. HPV vaccination, initially for girls, was extended in 2021 to boys aged 11-14, with catch-up vaccination up to 19, extended to 26 in 2025. Vaccination competencies were expanded to community pharmacists in 2022. The PAPILLON study used claims data to monitor HPV vaccination coverage rate (VCR) among 11-19-year-olds.
METHODS: In this French health claims database (SNDS) study, partial vaccination was defined as the dispensing of at least one HPV vaccine dose, and the full scheme vaccination as the dispensing of two or three doses of HPV vaccine over an 18-month period according to French recommendations. From 2021 to 2024, annual HPV vaccine initiation rates and cumulative VCR were assessed among adolescents aged 11-19 years. Proportion of HPV vaccinations and prescriptions involving pharmacist was estimated in 2024.
RESULTS: Overall, 1,528,792 girls and 1,369,167 boys initiated HPV vaccination between 2021 and 2024. In 2024, 72.2% of initiations occurred at ages 11-14 and 27.7% at ages 15-19. In 2024, the cumulative VCR for the partial scheme at 15 years was 61.7% for girls and 40.4% for boys. The cumulative VCR at 15 years for the full scheme increased between 2021 and 2024, from 32.5% to 42.5% in girls and from 0.8% to 24.5% in boys. Pharmacist prescribing remains marginal (<1%) in 2024, while their role in administration increases with age and dose number, reaching around 13% in 11-14-year-olds and over 30% in 15-19-year-olds for the third dose.
CONCLUSIONS: HPV VCR increased between 2021 and 2024 but remains below the target. A clear increase in HPV VCR among boys is observed, reflecting improved uptake over time. Our results highlight the importance of catch-up cohorts. Community pharmacists’ prescribing is underestimated, as the code became operational in October 2024.
