EQ-5D-Y-3L UTILITIES OF CHILDREN WITH MYOPIA: RESULTS FROM A REAL-WORLD INTERNATIONAL EUROPEAN SURVEY

OBJECTIVES: Myopia develops during childhood, often progresses throughout early life, and increases the risk of complications in adulthood. This analysis aims to quantify its impact on children’s health-related quality-of-life (HRQoL).
METHODS: Data were drawn from a real-world, cross-sectional, digital survey conducted among proxy respondents for children with myopia in Denmark, France, Germany, Italy, Spain, Sweden, and the UK between July 2025 and April 2026 (NCT06912802). HRQoL was assessed using the EQ-5D-Y-3L, supplemented with a “vision” bolt-on to capture vision-related impairment. Utility values were calculated using the Spanish EQ-5D-Y-3L value set. Utilities were evaluated across myopia severity categories, defined by the Spherical Equivalent Refraction (SER): low myopia (-0.5D to ≤-3.0D), moderate myopia (-3.0D to ≤-6.0D), and high myopia (< -6.0D). Multivariable linear regression analyses assessed the association of dioptre with utility values and VAS scores, adjusting for age, gender, and country.
RESULTS: Data from 238 children were analysed (mean age 11.0 years; 57.1% female), with a balanced distribution across severity groups (36.1% low, 33.6% moderate, and 30.3% high myopia). Children with high myopia more frequently reported moderate-to-severe problems across several EQ-5D dimensions compared with those with low-to-moderate myopia, including usual activities (25.0% vs 9.6%, p<0.001), feeling worried, sad or unhappy (37.5% vs 18.7%, p=0.008), and vision (53.6% vs 65.3%, p<0.001). Mean (SD) utility values and VAS scores were 0.951 (0.118) and 91.0 (12.8) among children with low-to-moderate myopia, compared with 0.877 (0.160) and 80.8 (19.2) among those with high myopia. Among children with high myopia, each dioptre point increase was associated with a 0.031 reduction in utility value (p<0.001). Overall, each dioptre unit increase was associated with a 1.2-point decrease in VAS score (p<0.001).
CONCLUSIONS: High myopia is associated with reduced HRQoL in children, particularly impacting usual activities and mental health, highlighting the need for treatments to slow down myopia progression.
