THE GROWING IMMUNOGLOBULIN SUPPLY-DEMAND GAP IN EUROPE: ASSESSING SELF-SUFFICIENCY TARGETS AND THERAPEUTIC ALTERNATIVES

OBJECTIVES: Demand for immunoglobulins (Ig) in Europe is expected to increase substantially with population aging, improved diagnosis, and the growth of patients with secondary immunodeficiencies, for whom Ig remains essential. Because Ig production depends on human plasma donations, supply is inherently constrained. European domestic production remains insufficient to meet current needs, resulting in import dependence. This study assessed the gap between self-sufficiency ambitions and performance in selected European markets and explored the contribution of therapeutic alternatives in reducing future supply pressures.
METHODS: A country-level self-sufficiency model projected Ig requirements and evaluated progress toward self-sufficiency targets in Belgium, France, Italy, Netherlands, Spain and United Kingdom, based on domestic plasma and locally manufactured Ig. Current and projected Ig demand and domestic supply data were derived from blood agencies, public reports, and international market sources. Demand-side scenarios examined partial substitution of Ig use in chronic inflammatory demyelinating polyneuropathy (CIDP) and myasthenia gravis (MG), two high-Ig-utilization indications with recently approved non-Ig alternatives.
RESULTS: Across the six markets, self-sufficiency targets ranged between 30% and 90%, while domestic Ig production averaged 38% (range: 23%-57%) of total consumption in 2024. By 2030, Ig demand is projected to increase by 59% (range: 35%-100%), compared with a 28% increase in domestic supply, further widening the gap between achieved and target self-sufficiency levels. CIDP and MG together account for 27% of total Ig utilization (range: 19%-30%). Clinically appropriate switching of eligible patients to alternatives could reduce Ig demand growth by 16 percentage points (range: 11-22), helping narrow self-sufficiency gaps.
CONCLUSIONS: Foreign Ig and plasma are likely to remain critical in the context of growing global demand, highlighting the need for strategies that support long-term supply security and sustainability. Complementing plasma collection initiatives with clinically appropriate use of non-Ig therapeutic alternatives may improve supply security and preserve Ig access for patients with limited therapeutic options.
