REAL-WORLD TREATMENT PATTERNS AND LINE-TO-LINE TRANSITIONS IN MULTIPLE MYELOMA IN POLAND: A POLICY-ALIGNED ANALYSIS OF PUBLICLY AVAILABLE NATIONAL HEALTH FUND (NFZ) DATA

OBJECTIVES: To describe real-world treatment patterns, line-to-line transitions, time-to-next-treatment (TTNT), and treatment discontinuation in multiple myeloma (MM) patients in Poland using publicly available National Health Fund (NFZ) data, and to assess their alignment with successive updates of the Polish Myeloma Group (PGSz) clinical recommendations.
METHODS: We performed a retrospective analysis of publicly available NFZ data, including the national report and its supplementary dataset covering January 2023 to June 2025. Administrative data from SMPT and SWIAD were analyzed in 6-month intervals. The analysis covered: (i) patient populations treated under drug programme B.54 and the chemotherapy catalogue; (ii) regimens and treatment lines (DVd, DVTd, DRd, EloPd, IRd, Kd, KRd, PVd, Pd, IsaPd, teclistamab, elranatamab, talquetamab); (iii) line-to-line transitions and TTNT; (iv) discontinuation reasons; (v) resource use; and (vi) refund value of services. PGSz recommendation updates were mapped onto the observed timeline to interpret shifts in real-world practice.
RESULTS: The analysis will describe MM treatment pathways in Poland, including changes in regimen mix across consecutive lines, line-to-line transitions, and TTNT trends. Discontinuation patterns will be quantified by reason and regimen to identify clinically relevant drop-out signals. Mapping PGSz updates onto the observed timeline will allow assessment of alignment between guidelines and real-world practice, and identify regimens where uptake lags behind recommendations. The resulting evidence may inform treatment sequencing assumptions, model validation, and contextual interpretation of effectiveness and resource use in HTA submissions.
CONCLUSIONS: Publicly available NFZ data, combined with PGSz recommendations, allow assessment of real-world MM treatment pathways and their alignment with clinical guidance. This approach may improve the contextual relevance of HTA in Poland and could be replicated in other disease areas with established drug programmes.
