OBJECTIVES: Within Türkiye’s International Reference Pricing (IRP) system, the Manufacturer Sale Price (MSP) of a pharmaceutical serves as the foundational external baseline. This study aims to conduct a comparative analysis of the average MSPs for original pharmaceuticals across selected benchmark nations, evaluating how external pricing levels vary by reference country.
METHODS: The analysis was conducted using the Detailed Pharmaceutical Price Lists published by the Turkish Medicines and Medical Devices Agency (TITCK) between 2021 and 2025. Original pharmaceuticals referencing Türkiye’s official IRP reference countries—France, Greece, Italy, Portugal and Spain—as well as Germany and Switzerland due to their frequent use in recent years, were included in the analysis. Non-reimbursed and over-the-counter products were excluded. Analyses were performed using Microsoft Excel.
RESULTS: Overall, 12,462 original pharmaceuticals were analyzed from 2021 to 2025, with annual counts of 2,832, 2,747, 2,362, 2,243, and 2,278 respectively. For the 2021-2025 period, average MSPs across reference countries were; France: €450.71, €590.42, €761.64, €729.41, €780.25; Greece: €285.22, €320.88, €343.36, €353.60, €320,62; Italy: €178.19, €151.51, €233.18, €231.18, €269.02; Portugal: €59.15, €57.31, €62.90, €48.18, €71.71; Spain: €38.69, €46.37, €44.58, €46.13, €66.20; Germany: €520.88, €696.70, €1,038.72, €1,239.56 €1,775.63 and Switzerland: €1,794.78, €1,432.35, €1,407.09 € 1,515.52 €1,665.26.
CONCLUSIONS: Average MSPs of original pharmaceuticals increased steadily from 2021 to 2025, driven by distinct external pricing dynamics. Switzerland maintained the highest pricing levels until 2024, when it was surpassed by Germany in 2025 due to a sharp increase. Conversely, Spain and Portugal consistently recorded the lowest prices, while France remained in the median tier. These substantial disparities demonstrate the pronounced variations in external pricing environments and underscore that reference country selection is a critical determinant within international pricing frameworks.
