OBJECTIVES: The aim of this real-world data analysis was to estimate the annual incidence of metastatic solid tumors in German adults and to describe antineoplastic treatment rates as well as survival probabilities.
METHODS: This retrospective analysis was based on anonymized nationwide claims data from statutory health insurances in the WIG2 database (~4.5 million insured individuals). Adult patients with newly diagnosed metastatic solid tumors during 2015-2023 were identified by a confirmed solid tumor diagnosis (ICD-10 C00-C75, C80) combined with a first-ever diagnosis of metastatic disease (ICD-10 C78-C79), defining the index quarter. Annual incidence was age- and sex-standardized to the German population. Antineoplastic therapy within one year of index was defined as ≥1 record of ATC L01 (excl. L01CH/CP/CX) or OPS 8-54 (excl. 8-548); survival from index was estimated by Kaplan-Meier. Outcomes were reported overall and by 3-digit ICD-10 tumor type. A look back period (1-9 years) was used to determine if the metastatic solid tumor was newly diagnosed.
RESULTS: 67,273 patients with newly diagnosed metastatic solid tumors, with a mean age of 69.2 years (standard deviation 12.1) at metastasis and a higher proportion of male patients (58.9%) were identified. Extrapolation to the German population indicated that the estimated number of incident metastatic solid tumor patients remained substantial, ranging from ~200,000 to 210,000 patients annually between 2015 and 2023. About half of patients received antineoplastic therapy within the first year after diagnosis, and this rate has remained stable over time. The one-year overall survival ranged from ~20% to ~80%, depending on tumor type.
CONCLUSIONS: Between 2015 and 2023, metastatic solid tumors affected a substantial number of adults in Germany, with claims-based extrapolation indicating 200,000 newly diagnosed patients annually. The finding that only approximately half received antineoplastic therapy within one year of diagnosis, together with substantial variation in survival across tumor entities, suggests a considerable disease burden.
