ECONOMIC BURDEN OF PNEUMOCOCCAL DISEASE OVER 2019-2022 ACROSS RISK PROFILES IN ADULTS: A FRENCH NATIONAL CLAIMS DATABASE STUDY

OBJECTIVES: Pneumococcal disease (PD), including invasive disease (IPD) and non-bacteremic pneumococcal pneumonia (NBPP), remains a major cause of morbidity and mortality in France. Vaccination recommendations historically targeted adults with underlying medical conditions (UMCs) and were expanded to all individuals aged ≥65 years in 2024. Describing the historical economic burden across age, disease types and risk groups, under both historical and newly expanded vaccination eligibility, can inform future vaccination strategies.
METHODS: A retrospective cohort study was conducted using the French national health data system (SNDS). Adults (≥18 years) hospitalized for PD over 2019-2022 were identified. Medical costs from the healthcare system perspective, including inpatient and outpatient care, were estimated at both the population and episode levels and stratified by PD type, age and risk group.
RESULTS: Over 2019-2022, 52,625 hospitalized PD episodes were identified in 51,247 adults: 82% were NBPP and 18% IPD. Most individuals (73%) had UMCs. Among those not previously eligible for vaccination (i.e., without UMCs; n=13,643), over half (51%) were aged ≥65 years. The mean (SD) cost per PD episode was €24,083 (€37,751). Costs were higher for IPD than NBPP (€27,217 (€36,777) vs. €23,409 (€37,924)) and among adults with UMCs compared to those without (€24,781 (€37,874) vs. €22,116 (€37,334)). At the population level, hospitalized PD episodes generated an average annual cost of €316.8 million. Adults with UMCs accounted for 76% of this burden (€240.6 million) despite being eligible for vaccination prior to the expansion. An additional 11% (€33.5 million) was attributable to individuals aged ≥65 years without UMCs, representing the burden of those who would be newly eligible under expanded age-based recommendations.
CONCLUSIONS: Based on pre-expansion data, a substantial share of hospitalized PD economic burden in France is concentrated among individuals eligible under both historical and newly expanded vaccination criteria, underscoring the importance of improving vaccination uptake to reduce the burden.
