BUDGET IMPACT OF INTRODUCING ACCU-CHEK SMARTGUIDE REAL-TIME CONTINUOUS GLUCOSE MONITORING (RTCGM) SOLUTION FOR DIABETES MANAGEMENT IN THE UK NHS

OBJECTIVES: Managing the growing burden of diabetes remains a critical priority for the UK NHS. While continuous glucose monitoring (CGM) has already transformed standard care, the Accu-Chek® SmartGuide solution builds on this, with predictive features designed to support users in anticipating potential hypo- and hyperglycemic events. This early analysis aims to estimate the 5-year budget impact of introducing the Accu-Chek® SmartGuide solution in the United Kingdom from an NHS perspective.
METHODS: A budget impact model (BIM) was developed in Microsoft Excel® to project the 5-year financial impact of introducing Accu-Chek® SmartGuide, comparing a "World with" versus "World without" the solution. The model evaluated UK adults with T1D or T2D treated with multiple daily injections. The analysis incorporated costs for blood glucose monitoring (SMBG, isCGM, rtCGM) and management, alongside the costs of acute events and chronic complications. Chronic complications were modeled using the IQVIA Core Diabetes Model, and acute event rates were informed via a targeted literature review. The benefit of Accu-Chek® SmartGuide on non-severe hypoglycemia was informed by retrospective real-world data for nighttime events and in-silico data for daytime events. Model costs were sourced from UK databases. For the purposes of this analysis, sensor costs were assumed equal across all CGM platforms. The model assumes Accu-Chek® SmartGuide captures market share from existing CGMs over time.
RESULTS: In the base case, introducing Accu-Chek® SmartGuide resulted in a total 5-year net budget saving of -£17.1 million (approximately -£42 per patient). Savings were primarily driven by reduced non-severe hypoglycemia and chronic complications costs.
CONCLUSIONS: Introducing Accu-Chek® SmartGuide in the UK has the potential to generate budget savings, primarily driven by its potential to reduce non-severe hypoglycaemia. Future integration of UK real-world evidence on the impact of Accu-Chek® SmartGuide on glycaemic outcomes will provide a more accurate assessment of budget impact.
