ADHERENCE TO THE ISPOR GLOBAL PHARMACOECONOMIC MODEL ADAPTATION GUIDANCE: A REVIEW OF PUBLISHED ECONOMIC MODEL ADAPTATIONS

OBJECTIVES:
Economic models developed in one jurisdiction are frequently adapted for use in other countries to inform reimbursement and HTA decisions. In 2014, ISPOR published the Global Pharmacoeconomic Model Adaptation Guidance, comprising 16 recommendations to support model transferability and local adaptation. This study assessed the extent to which published economic model adaptations adhered to these recommendations.
METHODS:
A targeted literature review was conducted in Value in Health Regional Issues. The search strategy combined terms related to transferability, adaptation, localization, multinational analyses, cross-country studies, and country-specific evaluations. Publications from 2014 onward, describing adaptation of an existing economic model, were eligible for inclusion. Original economic model development studies and review articles were excluded. Each included study was assessed against the 16 recommendations, and adherence was summarized as the proportion of recommendations met.
RESULTS:
The search identified 75 records, of which 18 studies met the inclusion criteria. Mean adherence was 89.7%, with 14 of 18 studies meeting at least 87% of all recommendations. Five recommendations were universally met: selection of local comparators, identification and quantification of resource use and costs, justification of multinational treatment-effect estimates, use of modelling for non-transferable elements, and translation of findings for local decision makers. The lowest adherence was observed for use of country-specific epidemiologic data (56%) and justification of each input parameters (64%). Only one study (Alshreef et al., 2019) explicitly cited the guidance. Two additional studies cited the CHEERS reporting guideline, whereas 15 cited neither.
CONCLUSIONS:
To our knowledge, this review provides the first empirical assessment of adherence to the ISPOR Global Pharmacoeconomic Model Adaptation Guidance. Published economic model adaptations were generally well aligned with the guidance despite only one study explicitly citing it. Consistent adherence to the guidance is essential not only to produce robust, locally relevant adaptations but also to enhance comparability across studies and jurisdictions.
